b) License to work a Lift / Escalator by Directorate of Electricity under Department of Power & Non-conventional Energy Sources, Government of West Bengal

1. Permission Letter to Erect a Lift.
2. Application Form B signed by Owner of the lift and countersigned by Erecting Firm with seal.
3. Safety Certificate regarding functioning of over speed governor, car safety unit, Buffer, Brakes, Terminal and Final stopping device and locking device.
4. Manufacturers Certificate regarding quality, material construction of main suspension ropes, insulation of electrical equipment, speed governor, key of driving sheave etc.
5. Wiring Diagram for control panel and Lift equipment.
6. Details of Motor, gear, pulleys, ropes etc. as installed in the lift.
7. Test form issued by the licensed electrical contractor on electrical aspect of the lift.
8. Wiring diagram of the control panel and the lift equipment.
9. Current Property Tax receipt for Proof of Ownership.
10. Sanctioned Building Plan.
11. Duly approved Layout drawing of the Lift. Layout Drawing should mention Drawing Number and Date of Approval of competent authority.
12. Self Declaration by owner indicating the layout drawing of Lift is made as per the duly sanctioned plan.
13. Self Declaration by Lift Erecting Firm indicating the layout drawing of Lift is made as per the duly sanctioned plan.
